I also usually pull the injectors fairly early in the head removal process, in order to catch any injector, cup, or head issues early in the job. However, we had replaced 3 of the injectors in the past couple of years and didn’t see any of these problems, so I waited until the end of the day for injector removal. The two problems cause so close to the exact same symptoms, there’s no way to be certain weather it’s cups or Gaskets (although every single one we’ve done in house you can see where the gasket was leaking, It still doesn’t mean a cup wasn’t also part of the problem… ) So do them both to be safe.

Remove the injector sleeve from the sleeve stud by cutting away the rolled-over portion. Use 6V-6193 or 1U-7228 Diagonal Cutter Pliers or similar tool to remove the rolled-over material. Clean the injector bore and prepare for sleeve installation as per Special Instruction, SEHS9246-01. Do not cut the hose until after you have installed it on the fuel injector. The hose is then cut at 1 1/2 inches. This applies only to my fabricated fuel fail. The purpose of this short hose is to lower my customized fuel rail sufficiently to clear the throttle kick down switch. Cut hose to the required length with a sharp razor blade. IF the injector cup isnt sealing AND the bottom o-ring on the injector failed at the same time, it is possible for diesel fuel to get into the cooling system. Im not saying thats what happened, but it is possible... If the bottom o-ring on the injector keeps failing then there is something wrong in the bore.
